You will need to provide the following before the license can be issued

1. Current copy of your State Health Department Inspection.
2. A copy of your Driver’s License.
3. Proof of Insurance.
4. City of Shepherdsville Food Truck Application filled out entirely.
5. Return all forms with $100 yearly fee payment to City Hall.
